Fracturas complejas de húmero proximal en pacientes laboralmente activos

Resumen

El tratamiento de las fracturas complejas de húmero proximal continúa siendo un reto para el cirujano ortopédico. Tanto el tratamiento clásico con inmovilización con cabestrillo y tratamiento funcional hasta las últimas técnicas quirúrgicas tienen una indicación muy relacionada con el grado de afectación del hueso y las necesidades funcionales del paciente.

Se describen las diferentes opciones para tratar este tipo de fracturas: abordajes quirúrgicos, por vía abierta, percutánea; con placas, clavos y prótesis. Cada una de estas opciones presenta una serie de indicaciones, pero debe ser el cirujano, en función del tipo de fractura, la demanda funcional del paciente y sobre todo su experiencia, el que deba decidir cuál es la mejor opción en cada caso, sabiendo que, aunque se intente sistematizar, siempre existirán excepciones a la regla.

Abstract

Complex fractures of the proximal humerus continue to be a challenge for the orthopedic surgeon, from the classic treatment with immobilization with sling and functional treatment to the latest surgical techniques, they continue to have their indication very related to the functional needs of the patient and the degree of injury of the bone. We present the different options to treat this type of fractures, different surgical approaches, using plates, nails and joint replacements. Each of these options presents a series of indications but it must be the surgeon, depending on the type of fracture, the functional demand of the patient and especially his experience, who must decide which is the best option in each case, knowing that, even if you try to categorize, there will always be exceptions to the rule.
