Resultado funcional esperable tras una artrodesis bilateral de tobillo. Valoración de 3 casos y revisión de la literatura

Resumen

Abstención, artrodesis, artroplastia o combinaciones de estas últimas son las opciones terapéuticas en artrosis bilateral de tobillo. En la indicación quirúrgica es razonable dudar por temor a una mala calidad de vida en caso de artrodesis bilateral. Los pocos estudios publicados se centran en artrodesis de un tobillo. El objetivo de este trabajo es conocer qué resultado funcional pueden esperar los pacientes tratados con artrodesis bilateral de tobillo. Se analizó una serie de casos de 3 pacientes intervenidos de artrodesis bilateral de tobillo en nuestro centro. Se evaluó el resultado funcional preoperatorio y postoperatorio según la escala AOFAS (American Orthopaedic Foot and Ankle Society) para retropié/tobillo y según la escala SEFAS (Self-Reported Foot and Ankle Score) para retropié/tobillo postoperatoria. El resultado funcional fue satisfactorio tras 7 años de seguimiento medio (rango: 5-8) con una puntuación prequirúrgica media de 55,6 frente a 73 puntos en la última revisión, según la escala AOFAS. El resultado funcional según la escala SEFAS fue de 36,6 sobre 48 puntos. En conclusión, en nuestra experiencia y según la bibliografía existente, la artrodesis bilateral de tobillo es una opción terapéutica que puede tenerse en cuenta para el tratamiento de la artrosis de tobillo con un buen resultado funcional.

Abstract

Abstention, arthrodesis, arthroplasty or combinations of the latter are the therapeutic options in bilateral ankle osteoarthritis. In case of bilateral arthrodesis is reasonable doubting of the surgical indications for fear of a poor quality of life. The few published studies focus on arthrodesis of an ankle. The aim of this study is to know what functional outcome might expect the patients treated with bilateral ankle arthrodesis. We analyzed a series of three case reports of patients who underwent a bilateral ankle arthrodesis in our center. The functional outcome was evaluated, as pre-operative and post-operative scores with AOFAS (American Orthopaedic Foot and Ankle Society) scale for hindfoot/ankle and with SEFAS (Self-Reported Foot and Ankle Score) scale post-operative for hindfoot/ankle. The functional outcome were satisfactory after 7 years of mean follow-up (range: 5-8) with an average pre-operative score of 55,6 versus 73 points in the last revision, according to the AOFAS criteria. The functional outcome according to the SEFAS scale was 36,6 out of 48 points. In conclusion, in our experience and according to the literature, bilateral ankle arthrodesis is a therapeutic option that might take into account for the treatment of ankle osteoarthritis with good functional results.
