Reconstrucción ósea en metástasis del tercio distal de tibia utilizando aloinjerto y enclavado retrógrado

Resumen

Presentamos el caso de un varón con carcinoma de vejiga de 16 años de evolución que acude a nuestra consulta por presentar metástasis en metáfisis distal de tibia izquierda y en donde se realizó una cirugía reconstructiva consistente en la resección del tumor y la utilización de un aloinjerto óseo intercalado. Se utiliza un clavo retrógrado como sistema de osteosíntesis.

Esta técnica permite una más rápida recuperación de la marcha y evita las secuelas de la amputación (depresión, miembro fantasma, alteraciones del esquema corporal, etc.). La seguridad en el control de la lesión local es semejante a la amputación y las complicaciones registradas son mayores desde el punto de vista estadístico pero pensamos que es una obligación del cirujano ofrecer soluciones al paciente que le permitan una mejor calidad de vida aunque padezca una enfermedad de mal pronóstico.

Abstract

We present [he case of a male that suffers a bladder carcinoma with an evolution of 16 years. He goes to our consultation because of metastasis in the distal left tibia and we carry out a reconstructive surgery consistent in the reseetion of the tumor and the use of an intercalary bone allograft. A retrograde nail was used as internal fixation system.

This technique allows a quicker recovery of the march and it avoids the sequels of the amputation (depression, ghost member, alterations of the corporal outline, etc.). The security in the control of the local lesion is similar to the amputation and the registered complications are bigger from the statistical point of view but we think that it is the surgeon's obligacion to offer solutions to the patient that allow him a better quality of life although he suffers an illness of bad presage.
