Summary

Introducción: las lesiones osteocondrales de talus (LOT) son poco frecuentes y pueden pasar desapercibidas inicialmente hasta en un 67% de los casos. La presentación habitual de un paciente con el antecedente de esguince de tobillo que consulta por dolor crónico con la carga y la movilización. Presentamos el primer caso publicado de reinserción artroscópica de un fragmento osteocondral rotado 180°.

Material y métodos: se presenta el caso de un varón joven que, tras una torsión de tobillo, presentó una LOT con el fragmento volteado 180° (cartílago articular en contacto con hueso subcondral del talus) que pasó inadvertida en el diagnóstico inicial de esguince de tobillo. Mediante artroscopia de tobillo se redujo a su posición anatómica estabilizándolo con dos tornillos biodegradables.

Resultados: el seguimiento clínico y las pruebas de imagen constataron la desaparición de los síntomas, buena integración del fragmento del astrágalo y congruencia articular anatómica. A los 24 meses de seguimiento presenta una puntuación de 90 en la escala AOFAS y un grado de satisfacción excelente.

Conclusión: la reinserción del fragmento es una buena opción de tratamiento, permitiendo restaurar la anatomía y recuperar la función previa rápidamente.

Abstract

Introduction: osteochondral lesions of the talus (OLT) are uncommon and may go unnoticed initially up to 67% of cases. The usual presentation is a patient with a history of ankle sprain consulting for chronic pain with loading and mobilization. We present the first case of arthroscopic reattachment of an osteochondral fragment rotated 180°.

Material and method: we present a case of a young male whom, after an ankle torsion, presented an OLT with the fragment rotated 180° (articular cartilage in contact with subchondral bone of the talus), which went unnoticed in the initial diagnosis of ankle sprain. The patient underwent arthroscopic anatomic reduction of the fragment and fixation with two bioabsorbable screws.

Results: clinical follow-up and imaging test confirmed the disappearance of symptoms, integration of the fragment and restoration of the articular congruity. At 24 months follow-up, the patient presents a score of 90 points in the AOFAS hind foot score and a hight level of satisfaction.

Conclusion: the arthroscopic reattachment of the osteochondral fragment is a good treatment option, allowing anatomy restoration and a quick recovery of the previous function.
